Tax document
A signed copy of your family's most recent 1040, 1040EZ, 1040A, or IRS tax transcript. Skip this if your family did not file.
Your School Counselor's email address
The application will automatically send a recommendation form to their email address to complete.
Email addresses for two different teachers
Each of the teachers will receive a recommendation form to complete.
Neither of your parents (biological/adoptive) has completed a four-year college degree.
Your family's taxable income falls within the federal income guidelines that is based on family size.

To request an application by mail, click the icon above and complete the form. We will send you a copy.
Once received, give your guidance counselor and teachers the recommendation forms.
Then fill out the application and return it in the enclosed envelope, along with tax returns or submit it to your school counselor.
Your school counselor will have copies of our application.
You will need to give your recommendation forms to your teachers and the guidance counselor.
Then return the completed application with the required documents to the school counselor.
